COVID-19 & MASKING GUIDELINE
Your care and safety are our top priorities. As we continue to provide the highest-quality patient care possible, we have implemented new safety protocols. These include but are not limited to, contactless patient registration procedures which require completing your patient paperwork prior to your appointment and having a credit card on file.
We understand that as individuals and as a community we have had to process many changes during the pandemic. Considering this, we have created a resource to help make your dermatology appointment as smooth as possible. New Patients
PATIENT REGISTRATION – COMPLETE THESE FORMS PRIOR TO YOUR APPOINTMENT
If you have already scheduled an appointment but have not yet filled out your Patient Registration Forms, please log into your Patient Portal to complete your forms online prior to your appointment.
If you are unable to complete your forms via the Patient Portal, please arrive fifteen minutes before your appointment to accommodate completing the paperwork which will be provided by one of our Patient Care Coordinators.
Please note that if you are an existing patient, you may be asked to complete a new set of Patient Registration Forms depending on the last time you had an appointment with us.
For security and verification purposes, our Patient Care Coordinators will also request a form of identification and your insurance card when you check in for your appointment.

Insurance
We provide courtesy billing to your primary and secondary insurance carriers. Insurance coverage for dermatology services varies depending on your insurance. Please check with your individual insurance carrier for information. Additionally, some plans require a referral from your primary care physician, so it is always a good idea to contact your insurance company for their specific requirements before your visit.
While we will bill most carriers on behalf of our patients, we cannot guarantee personal coverage or benefits. Only your insurance company can guarantee that our providers are in their network.
